Overview

KZ Therapy, a behavioral health clinic based in Portland, OR, intends to build a free practice management platform to achieve the following goals: 1. Incentivize and onboard thousands of behavioral health clinicians in solo and small group practices who, in exchange for using the platform, agree to predominantly serve Medicare and Medicaid members. 2. Lower barriers to accessing care for underserved, marginalized populations. 3. Improve quality of care/patient outcomes by leveraging ML and anonymized data-sets contributed by consenting users. These data will inform adaptive, evidence-based interventions, provide timely predictive analytics, and reveal meaningful hidden relationships.

Research Lead is a professor at Portland State University and holds masters degrees in Global Public Health from New York University (NYU) and Social Work from Portland State University (PSU).  She has a successful track record of implementing programs providing livelihood opportunities for marginalized populations.

The project will leverage Singularity's decentralized AI platform by utilizing various AI marketplace innovations including machine learning algorithms, knowledge graphs, privacy-preserving features, and applications related to sentiment analysis.  It will require Nunet's decentralized computing power.  By aligning provider and patient incentives and removing the centralized organizations behind existing platforms, power is shifted to platform participants.

We intend to disrupt the practice management SaaS behavioral health space and this is the first step.  With community support, Deep Funding can make access to mental healthcare possible for millions of people in need.  

The core problem we are aiming to solve

The core problem we will solve is 3-fold:

  1. Due to higher reimbursements rates, clinicians are incentivized to work with private insurance members instead of Medicaid-Medicare members.  The situation is especially dire for elderly folks with Medicare.
  2. Entrenched barriers to accessing care.  Presently, mental health practitioners cannot meet community demand in a timely manner (if at all), particularly for folks with minimal social protections, limited financial resources, and the elderly.  The majority (tens of millions!) of folks in the US diagnosed with a mental health condition are unable to access appropriate care. 
  3. Ambiguous quality of care and negative patient outcomes.  

Our specific solution to this problem

By offering free usage of the proposed SaaS practice management platform to behavioral health providers with caseloads consisting of 50% or more Medicaid-Medicare members, clinicians are incentivized to work with marginalized populations and address the shortfall of providers while simultaneously moving power from the private companies behind existing practice management solutions to clinicians and their patients.  As more clinicians and their patients use the platform, more data is fed to ML models which are continuously refined to produce more meaningful outputs.

By eliminating the expense of paying a hefty subscription fee - thousands of dollars annually - for a practice management platform such as SimplePractice, Theranest, etc., the proposed project tackles the incentive problem.  Solving the incentive problem also increases access, thereby removing a major barrier to access.  

Importantly, the SaaS practice management platform must provide a superior alternative to competitors and attract behavioral health clinicians of all stripes who can afford a subscription fee and effectively subsidize costs.  ML models will ensure needs-based resource allocation, facilitate therapist-patient fit, provide a first-of-its-kind adaptive human-AI hybrid treatment methodology to quantifiably improve patient outcomes.

Milestone 1 - Strategic planning document

Description

Completion of research and planning preliminary to building practice management platform and beta testing. The deliverable will outline detailed objectives for achieving project goals.

Deliverables

The deliverable will include essential functionality of the SaaS platform to support clinic operations similar to existing market players. Additionally it will describe how to integrate AI in order to glean insights from disparate data points to inform a first-of-its-kind human-AI treatment methodology based on relevant human and population-specific presentation symptoms diagnosis sentiment analysis and biopsychosocial ecology and markers. The deliverable document will explain necessary roles and expertise and requisite components for operationalizing.

    Thank you, Gombilla. You make some great points, and your concerns are valid and thought-provoking. Part of the work in the ideation/discovery phase of the project includes identifying a tech stack that will allow for maximum agility in terms of deploying future ML integrations while simultaneously ensuring privacy and security of PHI/ HIPAA-compliance. Mitigating components include data encryption, appropriate access controls, immutable audit trails, data segmentation and anonymization.  Additionally, informed consent, employee training, Business Associate Agreements with third parties, and audit schedules are prerequisites.  Quality and accuracy of ML algorithms with respect to personalized medicine in the field of mental health, will require time and experimentation to develop.  Moreover, rigorous testing and ongoing QA efforts will be required to tease out biases in data collection and analysis to ensure positive, ethical use.   In consideration of the concerns you raise about the nascent intersection of ML and PHI, we strategically plan to build a SaaS-based architecture with future integrations and updates in mind. However, our initial focus will be on core PM functions to ensure ease-of-use and workflow automation for clinicians and administrative staff. Initial testing will take place in our Portland-based clinic.  Subsequent beta testing will include a small representative sample of our target market, followed by pilots with non-profits working in underserved areas with members of Medicaid & Medicare.  We will pursue strategic alliances with trade organizations and industry publications to create awareness.

    Hey Nate. I must say that this is a well thought proposal and you have a solid team behind this. However, I sense possible concerns associated with the project could include ensuring data privacy and security, maintaining the quality and accuracy of ML algorithms, and addressing potential biases in data collection and analysis. Additionally, there may be challenges in incentivizing clinicians to use the platform and ensuring equitable access to care for all populations, particularly in underserved areas. I hope the team is keen on addressing these if they potentially arise?

    Obviously the free practice platform has encouraged doctors to serve more - I think this is also a good way to motivate through the application of advanced technology.     Experienced Proposers, Impactful Solution

    This Proposal will leverage an existing SNet service, from a Singularity Net Spinoff Company that is Nunet in the execution of its platform.

    The authors of the proposal seem to have a strong knowledge of the industry and the space they are working on, and this solution seems to solve the needs in that space. The authors are also using the built solution to align incentives for users, in these cases practitioners to provide support to poorer and lower-income populations, in dire need of mental health care. 

    With that goal in mind, the Team intends to offer the platform for free to practitioners attending lower-income populations, while at the same time seeking to attract other practitioners that would subsidize the costs. This model is interesting but complex, and not easy to make it happen. It can also hinder the eventual development and expansion of this product due to a lack of profitability and funding. 

    I like the idea of combining AI with human experts, for the supervision of the inputs and considerations given by AI, to limit any eventual negative impact the AI could have on the mental health of patients.

    Sounds like an interesting and Innovative Idea, the Team has a lot of experience in the area, although they will still need to find a good AI/ML expert to work with them in this research,

    As a health practitioner myself, I must say  this project has the potential to make a significant impact by addressing critical gaps in mental health care access for underserved populations. 

    I can also comment on the fact that incentivizing behavioral health clinicians to predominantly serve Medicare and Medicaid members through a free practice management platform, will lower barriers to accessing care and improve patient outcomes.

    Additionally, by empowering clinicians and patients with greater control over their data and treatment options, this project will promote a more patient-centric approach to mental health care delivery.
